Notice is hereby given that the South Central Colfax County Special Hospital District, Springer, New Mexico calls for proposals for: RFP # 2021-001
Roof Repair on Boiler Room/Fix & Repair Existing Sidewalks/Pour Additional Sidewalk & Concrete
The Board of Trustees for the South Central Colfax County Special Hospital District (SCCCSHD) is soliciting proposals for a company to provide roof repair services of a specific structure located at 615 Prospect Ave, Springer, NM. Repair includes fixing the roof of the boiler room, removal miscellaneous debris associated with the repair, and all materials associated with fixing the roof and its structure. Repair of existing sidewalk/concrete and pouring of new sidewalks/concrete includes all costs of materials and removal of any debris associated with its processes. Please provide an itemized proposal of all phases necessary to complete the project. The following requirements must be met:
• Professional References/portfolio of prior work
• Hold a valid Business License
• Hold a valid Certificate of Professional Liability insurance and business auto liability insurance covering
owned, hired and non-owned vehicles. Worker’s Compensation insurance, to cover obligations imposed by
Federal and State statutes pertaining to Contractor’s employees engaged in the performance of any
services, and Employer’s Liability Insurance. Contractor shall provide to SCCCSHD current certificates of
such coverage.
• Contractor must be a licensed GB-98 contractor
• Must provide State of New Mexico Taxation and Revenue Tax Identification Number.
• After contract award, all changes to the scope of work must have prior approval by the SCCCSHD CEO.
• SCCCSHD reserves the right to accept and/or reject all proposals or any portion of a single proposal. In ALL
cases, any award resulting from the RFP shall always be in the best interests of SCCCSHD.
Proposals will be received by South Central Colfax County Special Hospital District (Attention to: Kaycee Sandoval)
at 615 Prospect Ave., Springer, NM 87747 no later than Friday, April 23, 2021 at 2 pm. The company that is
awarded this contract will not be based solely on cost, but also on prior work history and portfolio.
The board will meet on April 26, 2021 at 7PM in Angel Fire to make the final award.
